SMIP-016, a new anti-tumour agent, is a mouse/human chimeric fusion protein built on the ADAPTIR™ (modular protein therapeutic) platform targeting human CD37. In this study, for the first time, we examined pro-apoptotic activity of SMIP-016 in combination with monoclonal anti-CD20 antibody, ofatumumab (HuMax-CD20) in de novo chronic lymphocytic leukaemia (CLL) cells and in different B-cell neoplasm-derived lines. In CLL cells SMIP-016 exerted significant cytotoxicity (versus control – p = 0.01). In the in vitro models, SMIP-016 was also distinctly active against Raji line (Burkitt lymphoma; BL) (versus control – p = 0.007), Riva-1 line (diffuse large B-cell lymphoma; DLBCL) (versus control – p = 0.002) and RPMI 8226 line (multiple myeloma cells; MM) (versus control – p = 0.03). In studies combining SMIP-016 and ofatumumab, the cytotoxicity against CLL cells was significantly higher than the agents used alone (p < 0.03). Remarkably enhanced cytotoxic activity of SMIP-016 and ofatumumab in combination was also observed in Raji and Riva-1 cell lines (p < 0.01 and p < 0.003, respectively). Importantly, both agents induced cytotoxicity at very low concentrations which suggests that potential side-effects may be decreased in clinical practice. The mechanism responsible for cytotoxicity of SMIP-016 in all the examined models was connected with caspase-dependent apoptosis. In majority of cell types SMIP-016 induced overexpression of Bax protein, as well as downregulation of Bcl-2, cIAP1 (p < 0.03) and Smac/DIABLO (p < 0.003) apoptosis-regulating proteins.

In conclusion, our study demonstrated high pro-apoptotic activity of SMIP-016, especially in combination with ofatumumab, against ex vivo CLL cells, and BL or DLBCL in vitro cell lines. Thus, further preclinical studies in in vivo models are warranted, as this combination may be a promising therapeutic concept for treatment of those malignancies.

Fait important, les agents induit une cytotoxicité à des concentrations très faibles ce qui suggère que les effets secondaires potentiels peuvent être diminuées en pratique clinique. Le mécanisme responsable de la cytotoxicité de SMIP-016 dans tous les modèles examinés a été relié à la apoptose dépendante de la caspase. Dans la majorité des types de cellules SMIP-016 a induit une surexpression de la protéine Bax, ainsi que la régulation négative de la protéine Bcl-2, cIAP1 (p <0,03) et Smac / DIABLO (p <0,003) des protéines régulant l'apoptose.

En conclusion, notre étude a démontré une forte activité pro-apoptotique de SMIP-016, surtout en combinaison avec l'ofatumumab, contre des cellules CLL ex vivo, et in vitro de lignées cellulaires BL ou dans DLBCL. Ainsi, d'autres études précliniques dans des modèles in vivo sont justifiées, car cette combinaison peut être un concept thérapeutique prometteur pour le traitement de ces tumeurs malignes.

The agent represents a new class of agents called small modular immunopharmaceuticals. Called CD37-SMIP, the agent targets a protein called CD37 on the surface of these leukemia cells.

L'agent représente une nouvelle classe de médicament appelé immunopharmaceutiques. Appelé CD37-SMIP cet agent cible uen protéine appelé CD37 sur la surface des cellules leucémiques.

The study shows that the agent can successfully attach to the protein on the leukemia cells and kill them. The agent works both by triggering the cells' self-destruction and by causing a particular class of immune cells to attack them.

L'étude montre que cet agent peut avec succès s'attacher à la protéine sur les cellules de leucémie et les tuer. L'agent montre que d'un coté il promeut l'auto destruction des cellules cancéreuses et de l'autre il incite les cellules du système immunitaire à attaquer ces mêmes cellules.

In an animal model, the agent worked equally as well as the drug rituximab, now routinely used to treat chronic lymphocytic leukemia (CLL) patients. Rituximab targets a different protein on leukemia cells.

Sur un modèle animal, l'agent a marché aussi bien que le rituximab, maintenant employé régulièrement pour traiter la CLL. Le rituximab cible cependant une protéine différente.

The study by researchers at the Ohio State University Comprehensive Cancer Center was published online in the journal Blood.

“Our findings have significant implications for the treatment of CLL and related malignancies,” says principal investigator John C. Byrd, director of the hematologic malignancies program at Ohio State 's James Cancer Hospital and Solove Research Institute.

Overall, Byrd says, “the findings indicate that this could be an effective agent for treating CLL and other malignancies, such as non-Hodgkin's lymphoma and acute lymphoblastic leukemia when they have expression of the CD37 protein.”

The laboratory portion of the study used CLL cells from patients, laboratory-grown non-Hodgkin's lymphoma cells and acute lymphocytic leukemia cells.

This research showed that the agent kills leukemia cells directly by triggering their self-destruction through the process of apoptosis.

The study also found that this self-destruction happens differently from how other drugs cause apoptosis. Most drugs cause cells to self-destruct by triggering a cell mechanism that requires enzymes called caspases. This new agent, however, works through a mechanism that does not require caspases.

“This is exciting because it means that this agent may benefit patients who are resistant to other CLL drugs,” says co-author Natarajan Muthusamy, a research scientist with Ohio State's Comprehensive Cancer Center. “It also suggests that it might work well in combination with other drugs, as well as alone.”

The findings also show that after the agent binds with the cancer cells, it attracts immune cells called natural killer cells, which also destroy the leukemia cells. Funding from the National Cancer Institute, the Leukemia and Lymphoma Society and the D. Warren Brown Foundation supported this research.
