Farnesyltransferase inhibitors are a novel class of anticancer agents that competitively inhibit farnesyltransferase. Initially developed to inhibit the farnesylation that is necessary for Ras activation, their mechanism of action seems to be more complex, involving other proteins unrelated to Ras. Of the four classes of farnesyltransferase inhibitors, at least three agents have been investigated in hematologic malignancies. Tipifarnib (R-115777), an orally administered non-peptidomimetic farnesyltransferase inhibitor, has shown promising clinical activity. Preliminary results from clinical trials demonstrate enzyme target inhibition, an acceptable toxicity profile and promising evidence of clinical activity. Ongoing studies will better determine the mechanism of action of tipifarnib and the role of combination with other agents, defining its place in the therapeutic arsenal of hematologic disorders.

Les inhibiteurs de Farnésyltransférase sont une nouvelle classe d'agents anticancéreux qui inhibent de manière compétitive la farnésyltransférase. Initialement développé pour inhiber la farnésylation qui est nécessaire pour l'activation de Ras, leur mécanisme d'action semble être plus complexe, impliquant d'autres protéines non apparentées à Ras. Parmi les quatre classes d'inhibiteurs farnésyltransférases, au moins trois agents ont été étudiés dans les hémopathies malignes. Tipifarnib (R-115777), un inhibiteur de farnésyl-transférase peptidomimétique non-administré par voie orale, a montré une activité clinique prometteuse. NEW YORK (Reuters Health) - When imatinib therapy has failed in patients with chronic myelogenous leukemia (CML) in chronic phase, adding the farnesyl transferase inhibitor tipifarnib restores activity against the disease and the combination is relatively well-tolerated, researchers report in the November 1st issue of Cancer.

Dr. Jorge Cortes and colleagues at The University of Texas M. D. Anderson Cancer Center in Houston treated 26 patients with chronic phase CML, half with Abl kinase domain mutations, with the combination of imatinib and tipifarnib, after an initial course of imatinib treatment failed.

The clinicians established that the maximum tolerated dose for the combination was tipifarnib 400 mg twice daily and imatinib 400 mg once a day. The median treatment period was 26 months
